GABC has selected Express Scripts to administer prescription drug benefits available under your medical plans. Express Scripts The procedure for accessing your benefit is simple: The insured or dependent receives a prescription from a physician and presents it along with their Express Scripts identification card to their participating pharmacist. The pharmacist will dispense the medication required and will request any co-payment under the terms of your benefit plan. Check your plan summary for details. When making a purchase using the Express Scripts card there are no forms to file or delays in receiving reimbursement.
What happens if a purchase is made without using the Express Scripts card? There may be occasions where a card has been lost or misplaced and the need for a medication is immediate. Contact GABC to report a lost card, remember a $5.00 replacement fee is assessed. In the meantime, an individual may pay the purchase price for a medication and request Direct Reimbursement. A reimbursement check will be issued by Express Scripts with a completed reimbursement form and prescription receipt.
What if my pharmacist is unable to obtain benefit information from Express Scripts at the time of purchase? Express Scripts operates a 24-hour Pharmacist Hotline to answer particular questions: 1-800-256-6134 Be sure your pharmacist has all information available on your identification card when he or she makes the call. Your Express Scripts sponsor number is YWE.
What about maintenance medications? Express Scripts operates a mail-order pharmacy service through CFI. Through this service an individual may receive up to a three-month supply of maintenance medications, including insulin. The information provided to all covered individuals contains a pre-addressed envelope for submitting maintenance drug orders to CFI. Additional forms may be obtained from Express Scripts at the following number:

insured will receive a plastic identification card (separate from
the Health Plan Identification Card) which will contain all of the
necessary information embossed on it. This will include the name
of the insured, identification numbers, dependent names (if covered)
and certain information about your benefit. Your pharmacist will
determine the level of coverage based on this information and from
the data received from Express Scripts in response to their request.
